But it's usually something like "what part of 'no' do you not understand?" - a formulation I'm guilty of using more frequently than I probably should, the implication being not that the other person is stupid but that he/she is inappropriately refusing to accept the 'no' (or whatever it is).

In my experience it isn't always quite as clear-cut. It's some statement that begs a question. "What part of 'Bxxxxt means Bxxxxt' do you not understand?" Or "What part of 'Lxxxe Means Lxxxe' do you not understand?" But I think I would also avoid "What bit of 'no' do you not understand?" I'd prefer, "I think I made myself clear: no." Or "I said 'no' and I meant 'no'." Or "In two syllables: no."
