I'd vote for commercial ad breaks within television programmes
In between programmes is just about bearable (i understand the need to pay the rent) and some of the ads are very creative but really they do give me the screaming hab-dabs within the body of a programme

In the same vein (and a more "modern" annoyance) - the "Next Time" spoilers at the end of episodes in serials before the end-credits run! So character just seen in peril of his/her life is immediately followed by a shot of him/her in the pub laughing![FONT=Comic Sans MS][I][B]Numquam Satis![/B][/I][/FONT]

1. Take one large chunk of cod-philosophy
2. Add several anodyne aperçus
3. Let it simmer and do plenty of navel-gazing (top tip: the Greeks called this 'omphaloskepsis'!)
4. Serve it up in a book and retail for £9.99
