Which 'K' links premieres in Milan, Salzburg, Salzburg (again) and Munich in a little over 10 years?

Dammit, just out of a meeting so I couldn't have a crack at that amateur special! Very clever with the Butterworth misdirection, Ams!!! I'd have been chasing down the banks of green willow like nobody's business!

Jez B's play is one of the best things I ever saw in a theatre...

Sounds and sweet airs, that give delight and hurt not.
        Sometimes a thousand twangling instruments

<< Mahler wrote his “Symphony of a Thousand” in a white heat of inspiration during the summer of 1906, and its 1910 premiere in Munich proved the greatest success of his career >>

Exhausted now. Any other offers? (K = 1000)

brilliant thinking Mr Flay, I'm just wondering how one work can have two premieres in the same city
