Well, I've got Marion Beeley, mother of Dennis Brain, who is variously described as a composer (who wrote the cadenzas for her husband Aubrey's performances of the Mozart Concertos) and a contralto who sang in a Proms performance of Elgar's The Crown of India;

And Crowns (of Thorns, Stars and metal) are mentioned in various Bach arias;

But I can't find any Crowns for Alans Bush, Hovaness or Ridout.

That's the complete set. I heard Alan Civil playing a couple of his cadenzas in a concert in about 1964. I was playing (second clarinet) in a mainly amateur orchestra, and he was the guest soloist in two concertos that evening - a Strauss concerto in the first part and a Mozart in the second. As there were no clarinets in the Mozart and there were two candenzas it must have been No 4. His cadenzas were remarkable for being very much his own, including some showing off with playing two parts (the first time that I had ever heard a horn player do that).

Ten years later I heard him playing the Third Concerto with the BBC Northern conducted by Raymond Leppard. The first time I'd seen a soloist empty the "drain" onto the wooden floor of the stage!
